Kansas Consent Forms
Comprehensive Health of Planned Parenthood Great Plains requires a 24 Hour Consent form that must be printed and signed 24 hours before any abortion procedure in the state of Kansas. Unemancipated minors seeking an abortion must additionally obtain notarized written consent of the minor and both parents (if the parents are married) or legal guardian(s) of the minor. Minors from Missouri must also complete an additional section at the end of the Notarized Minor and Parental Consent form, which is labeled "Notarized Attestation of Second Parent Notice," before their appointment.
Arkansas
If you are a minor who is coming from Missouri for an abortion appointment, the "Notarized Attestation of Second Parent Notice" form must be filled out before your appointment.
Oklahoma Consent Form
If you are under the age of 18, Oklahoma state law requires you to have one parent or legal guardian to consent before an abortion. Please print these forms and sign them in front of a notary prior to your appointment. Notary services may be available at the health center, but we recommend you complete this process prior to arriving at the center.
Missouri Consent Form
Comprehensive Health of Planned Parenthood Great Plains does not provide abortions in Missouri at this time. In certain circumstances, however, the State of Missouri requires that individuals seeking abortion services outside of Missouri be offered state-mandated materials. You can find those materials here. Please note that you do not need to review these materials or bring them to your appointment in Arkansas, Kansas, or Oklahoma.
